2018-2019 Transcript Request Form

2018-2019 Early Release and Late Reporting Form

  • Students and parent/guardians must complete an “Early Release/Late Arrival” application and have it reviewed and approved by their school counselor to participate in this program.

  • Students and parent/guardians must complete an “Early Release/Late Arrival” application and have it reviewed and approved by their school counselor to participate in this program.
  • If approved, students are not authorized to be on campus when they are not attending classes.
  • In order for the Early Release/Late Reporting request to be approved, all academic, athletic and attendance requirements must be satisfied.
  • All students are required to attend homeroom regardless of Early Release/Late Arrival status.
  • If your student does not comply with this agreement by failing to leave campus by 2:15 pm or arrive on campus before 9:45 am, an administrator may revoke this privilege by requiring them to be enrolled in a course.
  • If they fail to attend the course, they will be written up for skipping.

  • Late Reporting=No 1st Period Classes begin at 10:13 a.m. and end at 3:40 p.m.
  • Early Release=No 4th Period Classes begin at 8:30 a.m. and end at 1:59 p.m.

  • I understand that they will be required to furnish their own transportation for late reporting or early release.
  • I understand they are not allowed on campus without prior approval when not attending classes.
  • I understand that Cumberland County Board of Education does not carry insurance on students and their privately owned vehicles.
  • I acknowledge that it is my responsibility to assure that any vehicle that my senior is riding in is being driven by a fully licensed and insured driver.
  • I further agree to indemnify, hold blameless, release and waive the Cumberland County Board of Education, the individual members, agents, employees, and representatives thereof, as well as the program’s supervisors, from and against any claim which I, any other parent or guardian, sibling, the senior or any other person, firm or corporation that may have or claim to have, known or unknown, directly or indirectly for any losses, damages or injuries arising out of, during or in connection with the senior’s late reporting or early release.

(All Athletes Must Have Mr. Barbour’s Signature)For athletic eligibility purposes, Peer Helping does not count as one of the three academic courses needed to meet eligibility requirements. To meet eligibility requirements you must have 3 academic courses the prior semester. (If you are a spring athlete you must have 3 academic courses in the fall).

2018-2019 Early Graduation Intention Form 

  • Students and parent/guardians must complete a “2018-2019 Early Graduation Intention Mid Year Form” application and have it reviewed and approved by their school counselor to participate in this program.
  • Deadline Has Now Past.

This form must be signed following persons:

  • Student
  • Parent and or Guardian 
  • Counselor
  • Principal

Your child has opted to graduate early during his/her senior year, provided that all graduation requirements have been met by ________. All students are encouraged to remain in school for the full year, but if you and your child feel that mid-year graduation will fit your needs, we request that you and your senior verify this decision by signing the attached sheet and returning it to the counseling center at South View High School no later than _________. The following stipulations apply:

  1. Your child will be registered for four classes instead of eight for the

  3. Your child’s class rank will be based on first semester only. He/she will not be put back into the rank in June, therefore they will not be considered for valedictorian or salutatorian.
  4. Your child is responsible for obtaining information relating to seniors such as ordering caps, gowns and This is done entirely through contacting Jostens (www.jostens.com). He/she should keep in contact with a continuing senior at South View for information. The faculty, including counselors, is not responsible for notifying early graduates of senior information.
  5. Your child’s transcript will be evaluated for NC Scholar, Honor Senior and/or Presidential Academic Fitness Awards for graduation purposes based on their final transcript in January
  6. Your child will be able to attend the prom if in good standing when leaving South View High School in January 2018 AND if they have paid their prom If they are planning to invite a guest to the prom, then the appropriate guest application and fee must be paid. All school rules and dress codes apply to early graduates and their guests. Students who are interested in attending prom must receive approval through Ms. Maldonado in the main office.
  7. All media fines and other outstanding fees to include senior dues must be cleared prior to ________.
  8. Mid-year graduates will not receive their actual diploma until the entire class graduates in May 2019.

 Our transcript clerk can notarize this form for you as long as the parent is present with a valid ID – cost will be $5.00 per signature.
